OFFICE OF THE CIVIL REGISTRAR

 

I. Steps or Procedure, to Apply Birth, Marriage, and death Registration :

 

A.     Timely Birth Registration :
1.     Secure a  barangay certification where the birth occurs, certified true and correct by the Barangay Captain.
2.     Present said certification to either the Barangay Secretary for the preparation of the document or go directly to the Office of the Local Civil Registrar
3.     Get the owner’s copy of the document right after the registry number has been entered in the registry number box, duly signed by the Civil Registrar.

 

B.     Marriage Application
Local/Foreign Applicants :

 

1.     Secure certification from the barangay where the applicants reside.
2.     Present certificate of Live Birth of both male and female applicant
3.     If one party is a foreigner, a legal capacity to marry is to be presented.
4.     Bring CENOMAR- Certificate of No Marriage Records issued by the OCRG-NSO, Quezon City.
5.     Bring/present certificate of pre-marriage counseling from the priest, minister of their religious affiliation.
6.     Give the applicants copies of the marriage application to the priest or ministers where they wanted to be solemnized.
7.     Get their marriage license from the LCR office 10 days after the application, and present a copy of the POP-COM’s pre-marriage counseling certificate.

 

 

C.     Death:

 

1.     Secure a certification for death from the Barangay Captain where the death occurs.
2.     Present said certification to the LCR Office for typing or preparation.
3.     Let the attending Physician certify as to the cause of death. If confined in the hospital. If not confined, let the Municipal Health Officer assigned certify as to the cause of the death.
4.     Let the embalmer (if embalms) signed the portion at the back of the document.
5.     Get a copy of the document after the Registry Number Box, duly signed by the Civil Registrar.

 

Note: For a delayed registration of birth, marriage and death:
1. Affidavit for delayed registration, from a lawyer or anybody authorized to issue.

Search for Miss Teen Tambulig 2008 -

        Last January 14, 2008 the Municipality of Tambulig celebrates its Araw ng Tambulig with the Search for Miss Teen Tambulig 2008 as the main showcase of the celebration. There are eight locally discovered lovely teens who joined the search, namely: Princess Marjorie Castillo of Cluster 8, Cristy Faith Arbas of Cluster 5, Raiza Mae Bation of Cluster 1, Trecia Eve Cortes of Cluster 4, Happy Dream Lumingo of Cluster 2, Donna Jill Rodrigo of Cluster 3, Jamaica Pearl Micabalo of Cluster 6 and Kristia Eve Diwag of Cluster 7.

         Aside from the search there are also other events such as the BRGY OFFICIALS, LGU OFFICIALS AND EMPLOYEES NIGHT, FINAL JUDGING OF THE CLUSTER BOOTH, U CAN DANCE CONTEST, DUET COMPETITION ,SOFTBALL INVITATIONAL COMPETITION, and LIVE BAND as the finale of the celebration.

        The success of the celebration of Araw ng Tambulig 2008 was through the effort and leadership of our very own MAYOR CARIDAD B. BALAOD and the untiring support and cooperation of the LGU Officials, BRGY Officials, Local and National Employees and to the people of Tambulig as a whole.

Message

    As mandated by the Local Government Code of 1991 (RA 7160), through MC 2000-169 dated December 29,2000, that every LGU should implement the Local Governance and Performance management System (LGPMS), as one of the tools designed to assess and determine the capabilities and limitations of every Local Government Units, in the performance of their roles, functions and responsibilities, for the efficient delivery of basic services and for the effective public service, wherein the DILG is the direct oversheer of the program.

    For a better service-delivery performance, the LGPMS has given the policy-decision makers of the LGU of Tambulig, Zamboanga del Sur, a performance standards intended to measure the efficiency, effectiveness and quality of services rendered to the clientele, being the end-users of this measurement system.

    This LGPMS is a collective effort of endeavors to those responsible for the outcome whose emphasis is focused on the following performance measurement namely : Governance, Administration, Social Services, Economic Development and Environmental management.

    The LGU being a quasi-corporation is always measured by the political will of the Local Chief Executive, whose plans and programs is parallel to the approaches of the Legislative Branch for a common agenda and for the total attainment of the desired goals, wherein the LGPMS is the benchmark of quality measurement.
